California Tv station gets the names of the crew of that crashed plane in california wrong.  Wrong info came in from the NTSB and was run on air. 

I worked in a TV newsroom for years and that sort of thing would go through four or five people at least, the reporter, the news anchor, the person responsible for vetting (checking accuracy) of the story and finally the person who typed up the graphics on the station chyron.

surely one of them should have caught this.
